Companion Cards
Companion cards promote fair ticketing for people with a disability who have a lifelong need for the significant assistance of a companion to attend venues and activities. Call 13 13 04 or visit www.companioncard.asn.au (select Queensland) for further information.
Congratulatory Messages
Request a congratulatory message – 90 or 100th Birthday, 50, 60, 65, or 70th Wedding Anniversary by filling out the congratulatory message form. A copy of a Birth Certificate or Marriage Certificate will be needed. Alternatively, a statutory declaration can be completed.
Document witnessing
The office of Alex Douglas MP has a Commissioner for Declarations who is available to sign documents – 4/12 Ferry Street, Nerang. Please call 07 5502 1411 to make an appointment.

Statutory Declaration
A statutory declaration is a written statement declared to be true in the presence of an authorised witness. You can dowload the statutory declaration form here and there is an authorised witness at Alex’s office at 4/12 Ferry Street, Nerang. Call to make an appointment 07 5502 1411.
